The Chief Minister Jan Arogya Yojana is a state-level health insurance scheme run by the Uttar Pradesh government. Health Facilities Provided Under the Scheme
Under this scheme, every eligible family receives health coverage of up to ₹5 lakh annually. Treatment is completely cashless, meaning the patient or their family does not have to make any payment at the hospital. This includes heart surgery, angioplasty, cancer treatment, chemotherapy, kidney transplant, brain tumor, neurosurgery, maternity services, and other complex illnesses. More than 1500 medical packages determined by the state government are covered under this scheme.

Who Can Avail the Benefits of the Scheme?
Only permanent residents of Uttar Pradesh can avail the benefits of the Chief Minister Jan Arogya Yojana. Registered laborers working in the unorganized sector, BPL families not included in the SECC 2011 data, destitute elderly, disabled, and helpless citizens are considered eligible for this scheme. The government aims to extend healthcare services to those sections of society who were previously unable to afford expensive medical treatment.

Documents Required for the Scheme
To apply for the scheme, an Aadhaar card, a ration card, a residence certificate, an income certificate, a mobile number, a passport-size photograph, and a birth certificate are required. Eligibility is verified based on these documents, after which the beneficiary is included in the scheme.

How to Apply for the Chief Minister Jan Arogya Yojana

Both online and offline options are available for applying to this scheme. For the online application, you need to visit the official website and enter your information in the Chief Minister Jan Arogya Yojana section. After verifying eligibility through your Aadhaar or ration card number, the registration is completed. For offline applications, applicants need to visit their nearest public service center and submit all the necessary documents. Once the process is complete, the beneficiary receives a Golden Card, which entitles them to free medical treatment.
