How to make Ayushman Bharat card?

To get Ayushman Bharat card made, you can follow the following steps:

1. Online Application:

Visit the official website of Ayushman Bharat Scheme https://bis.pmjay.gov.in/BIS/selfprintCard.
Click on “Am I Eligible” option.
Enter your mobile number and verify it using OTP.
Select Ration Card, Aadhar Card or Location in “Search by” option.
Enter your information and click on “Submit” button.
Verify your identity (Aadhaar OTP, fingerprint, iris scan or facial recognition).
Click on the “Submit” button and submit your application.

2. Offline Application:

You can also apply offline by visiting your nearest Public Service Center (CSC) or Ayushman Bharat Seva Kendra.
Fill the application form with required documents (Aadhar Card, Ration Card, Income Certificate etc.).
Submit the application form and receive your receipt.
