Even a minor discrepancy in basic information such as name, address, or gender can result in your application being rejected immediately.

Verification fails if the father’s name on the Aadhaar card and the name in the source data differ.

The card will not be generated if the gender information between the Aadhaar and source data is different.

Any error or discrepancy in the date of birth record invalidates the application.

Live Photo and e-KYC Mandatory

A special campaign is underway in Uttar Pradesh to issue Ayushman cards to senior citizens above 70 years of age and family members identified but left out for the Ayushman Bharat scheme, starting November 25th and continuing until December 25th, 2025.
