Ayushman Card Rejected? These 5 Common Aadhaar Mistakes Could Be the Reason

The Ayushman Card is the most important document for this scheme, but even a small discrepancy in details can lead to your card being rejected. All information entered when making an Ayushman Card must match exactly with the Aadhaar data and the source data. If the data does not match at any stage, the application may be rejected. 5 Major Reasons for Ayushman Card Rejection

All information requested for the Ayushman Card on the beneficiary.nha.gov.in portal is verified directly with the Aadhaar records and the source data. Therefore, the following 5 errors can lead to rejection:

Even a minor discrepancy in basic information such as name, address, or gender can result in your application being rejected immediately.

Verification fails if the father’s name on the Aadhaar card and the name in the source data differ.

The card will not be generated if the gender information between the Aadhaar and source data is different.

Any error or discrepancy in the date of birth record invalidates the application.

If your address is different in Aadhaar and different in the source data, your application will be rejected.

Live Photo and e-KYC Mandatory

A special campaign is underway in Uttar Pradesh to issue Ayushman cards to senior citizens above 70 years of age and family members identified but left out for the Ayushman Bharat scheme, starting November 25th and continuing until December 25th, 2025.

According to information provided by the official handle of AB-PMJAY-UP, only a live photo, i.e., a photo taken at the time of birth, of the eligible beneficiary will be valid during Aadhaar e-KYC. Using any other or older photo may invalidate the card and result in rejection of the application.
