It can include the Gazette notification or marriage certificate if the address proof (eg Aadhaar card, electricity bill), birth certificate and if the legal process of changing the name is adopted.

For the improvement of passport, first of all, go to the official passport service portal (passportindia.gov.in) or log in. Now select the ‘Re-Eshu’ option and select ‘Name Correction’ here.
Next, fill the online form, giving new correct name and other necessary information. Carefully upload the scan copy of the appropriate documents and pay the fee.
Now book an appointment in Passport Seva Kendra or Passport Office.
What is the process ahead?
Print the application receipt (with ARN) after fee payment and appointment booking.
Go to the Passport Seva Kendra on the due date and take all your original documents together. The process is completed after examining the documents there.
If all the documents are found to be correct, a new passport is issued in which the correct name is mentioned. This entire process is transparent and can also be easily tracked.
