What is Ayushman Card?

How to get Ayushman card made?

To get an Ayushman card made from your phone at home, first of all you have to:

  • You have to install the Ayushman app on your phone. This is a government app.
  • After installing this app, you will have to choose the language of your choice.
  • After this, after clicking on login, you will have to click on Beneficiary.
  • After this you will have to fill captcha and mobile number.
  • After this, the page of Search For Nemeficiary will open in front of you. Here you have to select PM-JAY in the scheme.
  • After this you will have to select your state and district and by entering your Aadhar number you will be able to login to the app.
  • You will see the Ayushman cards of your family members in the app. You will see Authenticate written in front of the names of those who do not have an Ayushman card.
  • You will have to enter the Aadhaar number of the member by tapping on the Authenticate written next to his name and then you will get an OTP. After entering that OTP, a photo of the member will be clicked.
  • After this, you will have to fill in the member’s mobile number and his relationship with you. In this way, after completing e-KYC, you will have to submit the form.
  • After all the information is verified in a week, you will be able to download that member’s card from this app.

Required Documents

The documents required to get Ayushman card include Aadhar card, ration card, mobile number and passport size photo. Apart from this, if you have e-shram card, labor card or other government identity card, then they can also prove whether you are eligible for this card or not.
