SIR Enumeration Form: The Booth Level Officer (B.L.O.) may have already come to your home with the SIR enumeration form. It is very important to fill this form correctly because even a small mistake can lead to rejection of your application. You should know all the details about this new process of the Election Commission. Let’s understand step by step how to fill this form in a simple way.
The entire form is mainly divided into three parts, which every voter has to fill.
Step 1: Basic Details
At the top of the form, there is a space to write your name and address, though this part is mainly for BLO use. Next to it, you will find spaces for your serial number, part number, and a unique QR code.
Two important things in this section are:
- Old Photo: You need to paste your old voter card photo here.
- Recent Photo: Next to it, paste your latest or current photo.
Step 2: Important Personal Information
In this section, you must fill in your personal details carefully.
- Date of Birth: Write your correct date of birth.
- Aadhaar Number: It is optional, not mandatory.
- Mobile Number: Provide your active mobile number.
- Father’s and Mother’s Name: First, write your father’s name and his voter card (EPIC) number. Then write your mother’s name and her EPIC number.
- Husband’s Name: If your parents are not alive, women applicants can provide their husband’s name here. If the husband has a voter card, his EPIC number must also be written.
Step 3: Final and Most Important Stage
This is the last and most important part of the form, which is divided into two sections — left and right.
Left Side of the Form:
In this section, you need to mention the name of a voter whose name was in the 2002 revised list and state your relationship with that person.
- Write your name and EPIC number.
- Mention the name of your relative whose name appeared in the 2002 list. This relative can be from your father’s, mother’s, or grandfather’s side.
- Write your relationship with that person (for example – father, mother, grandfather).
- Mention the district, assembly constituency, and part number of that voter.
Right Side of the Form:
If your name is not in the 2002 revised list and you have mentioned a relative’s name on the left side, then you must fill in all the details of that relative again here — including their name, EPIC number, and relationship.
When Can the Form Be Rejected?
The Election Commission has mentioned two main reasons for form rejection:
- If you are not a citizen of India.
- If any information provided by you is false or misleading.
In such cases, your form will be rejected, and the Commission may also take legal action. So, if you are unsure about any detail, it’s best to ask the BLO (Booth Level Officer) for help before submitting.
