Offline Method
Step 1
1. If you meet the eligibility criteria, you can apply for the Ayushman card.
2. Head to your nearest CSC center.
3. Speak with the designated officer there.
4. They will verify your eligibility.
5. Your documents will be checked by the officer.
6. Once everything is confirmed, your application will be processed.
7. After a short while, your Ayushman card will be ready for download.
8. With this card, you can access free treatment up to Rs 5 lakh each year.
Online Process
1. Start by heading over to the official Pradhan Mantri Jan Arogya Yojana website at https://bis.pmjay.gov.in/BIS/mobileverify and click on the ‘Am I Eligible’ option in the menu.
2. You’ll be redirected to the Beneficiary NHA portal. Select the ‘Beneficiary’ option and input your mobile number. Then, fill in the captcha code and enter the OTP that you’ll receive on your phone.
3. Next, choose PMJAY as the scheme and provide the necessary details like your state and district.
4. Under the Search By section, pick ‘Ayushman Card’ and enter your Aadhaar number.
5. You’ll see the status of the Ayushman card for all family members listed along with their names.
6. If your Ayushman Bharat card shows as “Not-Generated,” go to the “Action” column and click the “Apply Now” button.
7. After that, authenticate yourself using your Aadhaar by entering the OTP sent to your registered mobile number.
8. Fill in the required information for your family members, including their mobile numbers, and submit everything to complete the online application for the Ayushman card.
9. Once your application is approved, you can download your Ayushman Bharat card.
